1. NSO/PSA Birth Certificate
2. Transcript of Records (TOR) with scanned picture and remarks “For Board Examination Purposes”
3. Marriage Certificate (if applicable, for married female applicants)
4. 2×2 ID pictures with white background and name tag
5. Community Tax Certificate (cedula)
6. Valid government-issued ID (Passport, Driver’s License, UMID, etc.)
7. Certificates of practicum or internship completion
