Losing your Professional Regulation Commission (PRC) ID can be stressful, especially since it’s more than just a piece of plastic it’s proof that you’re a licensed professional in the Philippines. Whether you’re a nurse, teacher, engineer, or any other regulated professional, your PRC ID is often required when applying for jobs, renewing licenses, or even traveling abroad for work.

The good news is that the PRC has a clear process for replacing a lost ID, and with the online system (LERIS), it’s easier than ever to request a duplicate. In this guide, we will walk you through the step-by-step PRC ID replacement process, including requirements, fees, and important reminders.

Why You Need to Replace a Lost PRC ID

Your PRC ID is an official government issued identification card. It serves several purposes:

Proof that you passed the licensure exam and are a registered professional
Requirement for employment, especially in healthcare, education, and engineering
Needed for license renewal and Continuing Professional Development (CPD) monitoring
Valid government ID for transactions like banking, passport applications, and legal documents

Losing it may cause delays in your career or personal transactions, so replacing it immediately is essential.

Here is the list of steps to get a Replacement Registration Certificate.

Step 1: Prepare the Requirements

Before applying for a replacement, make sure you have these ready:

  • Duly accomplished PRC ID Replacement Form (available on PRC’s website or at the PRC office)

  • Valid government-issued ID (passport, driver’s license, SSS, UMID, etc.)

  • Affidavit of Loss (notarized, explaining when and how your PRC ID was lost)

  • Passport-size photo with white background, formal attire, and full name tag

  • Proof of payment for replacement fee

 

Step 2: Apply Online Through PRC LERIS

Most PRC transactions now start online through the Licensure Examination and Registration Information System (LERIS).

  1. Go to https://www.prc.gov.ph/replacement-registration-certificate

Fill up the Form and Pay the Prescibed Fee at the Cashier and Follow the steps as per Official Website of PRC.

Step 3. Present duly accomplished AS together with the Affidavit of Loss/Damage and receive claim slip at: Window 16, Window 18 and Window 30.

Step 4. Claim your documents as scheduled. Please refer to your claim slip for further instructions.

Affidavit of Loss/Damage
Official Receipt
Passport size picture with name tag in white background

Step 4: Print and Bring Your Documents

Once payment is confirmed, log back into your PRC LERIS account and print the Application Form. On your appointment date, bring the following:

Printed application form
Notarized affidavit of loss
Valid ID and photocopy
Passport-size photo (if required)
Payment receipt

Step 5: Visit the PRC Office

On your scheduled appointment:

  • Submit your documents at the assigned PRC window
  • Have your photo and biometrics captured (if needed)
  • Wait for verification and approval

If everything is in order, you’ll be given a claim stub or release date for your new PRC ID.

Step 6: Claim Your Replacement PRC ID

Processing usually takes 2 to 4 weeks, depending on branch workload. On your release date:
